Versioned maintenance, not quiet revision

Insight Quantix notes are maintained as living analytical references. That means changes are documented by type: minor formatting fixes may be corrected silently, updated price/policy/benchmark assumptions increment the analytical version, and changed methods, thresholds, or governing conclusions require a major version with the earlier result preserved in the changelog.
